With several chronic conditions at once, advice, appointments and medicines can conflict. Person-centred care first identifies what matters most to the patient and creates one coordinated plan.
- Symptoms involving different organs, such as pain, breathlessness, fatigue, dizziness and low mood.
- Difficulty identifying which condition or medicine is causing a new symptom.
- Keep one up-to-date record of conditions, medicines, allergies, treating clinicians and important appointments.
- Prepare for a consultation by asking: what do I most want to be able to do or improve today?
- Make an appointment if new symptoms, side effects or conflicting advice disrupt daily life.
- For urgent GP care outside office hours, call HAP 9252. Call Ambulance 912 only for an immediate threat to life.
